rapid access chest pain clinic team has taken the initiative to solve the problem of rising referrals and lengthening waits. by changing the way they work, waiting times have tumbled and patients are benefitting from safer care. “it’s been a concerted effort by the chest pain team, supported by the cardiology consultants, booking office and cardiac department,” explains chest pain support practitioner, will delacour. “through refining the way we work and redesigning parts of the patient pathway we’ve been able to get waiting times back down to just under two weeks.” audits of the team’s patient activity identified a number of areas where they could make improvements. this included minor changes to gp referral forms to enable more accurate vetting of referrals and reducing unnecessary diagnostic investigations to make more efficient use of clinic time. “we no longer book all patients into a ‘one-stop-shop’ style clinic,” says will, “as many patients who didn’t need additional investigations meant we were wasting valuable scanning time. by booking these separately, we can now make sure we fully utilise appointments for investigations, ensuring every clinic is full and we also see far fewer dnas.” the team has also been smart about planning capacity around the christmas and new year period, running additional clinics in the weeks before to compensate for any that would be lost over the holiday period. these additional clinics were facilitated by staff having no annual leave in the two week christmas period. “it has meant this well-led team has been ideally placed to start the new year,” says associate director of nursing for medicine, liz trew. “they have worked amazingly well, picking up a problem and taking the initiative on finding their own ways to resolve it and to improve safety for patients.” it is now easier for the rapid access chest pain clinic team to expedite patients at higher risk of a serious cardiac event. they have also been working closely with colleagues in the emergency department to ensure patients seen there as emergencies are appropriately referred. will added, “our efforts have been aided by a small drop in referrals and we will continue to monitor our waiting times closely and work flexibly to maximise our service provision. a really big and genuine `thank you` to all involved in getting us back on track.” -- latest news rcht charity 5 yr old olivia’s thank you to hospital team may 15, 2018 | web team after olivia pascoe spent three weeks on the children’s ward at royal cornwall hospital with septic arthritis her parents wanted to say thank you for the care provided by the team. family and friends joined together to hold a fundraising event at portreath surf life saving club back in march, with olivia’s grandad, ken pascoe being a life member of the community centred club. funds raised were donated to royal cornwall hospitals charity to benefit the polkerris ward. “i couldn’t believe how much we raised. we had a fundraising target in mind which we smashed! everyone’s generosity was over and above and anything we could have expected,” said darren. the event, attended by over 200 people, started in the afternoon with family activities and a visit from the tolvaddon fire station crew. fundraising continued into the evening with food and drink a raffle of over 60 items donated by local businesses and music provided by locals nick williams, kaj skjervik and finlay barber on the drums. laura allsworth, event organiser and family friend, explained how the special community spirit of the village of portreath really contributed to the success of the fundraiser, “all generations of the village came together to make the day so successful, it is great that we still has such a connected community that all come together on these occasions and they are great fun!” said laura. olivia, a pupil at treleigh school in redruth, returned to polkerris ward to hand over the donation of £2012.80 and her parents, darren and becky were able to say a personal thanks to some of the team that cared for their daughter. “the whole team were amazing, the doctors, nurses, play specialists and even the lady that brought around the lunches,” said mum, becky. dan worrin, charge nurse, accepted the donation on behalf of the team and explained how their contribution would be put to good use, “we are so grateful for donations to our fund, together they allow us to buy additional pieces of kit that enhance the care that we provide to patients. thank you to everyone that supported olivia’s fundraising we are very grateful for your kind support,” said dan. thanks are given to everyone that supported the event and to local businesses that donated to the fundraiser. cornish | web team an exciting new initiative developed by nhs kernow clinical commissioning group, royal cornwall hospitals nhs trust, cornwall foundation trust and cornwall council to create a multi-agency hub to support people in cornwall who are at risk of experiencing a mental health crisis has been awarded £1.5m funding from the government. cornwall’s impact (integrated multi-agency prevention and assessment of crisis team) hub is one of 51 projects across the country to be awarded a share of £15 million announced by the department of health and social care as part of its beyond places of safety scheme. the national scheme provides funding for clinics, crisis cafés, and other community services designed to prevent people from reaching crisis point, as well as develop new approaches to support those who do. launched by the prime minister last year, the beyond places of safety fund follows the success of a previous scheme which helped to develop hospital-based places of safety for people experiencing a crisis to avoid them ending up in police cells. since the start of the scheme the numbers of detentions in police cells under the mental health act has fallen by more than 90 percent. the successful bid was developed by rcht through cornwall’s crisis care concordat steering group. chaired by dr paul cook, nhs kernow clinical lead for mental health, the group aligns local and national priorities, including peninsula-wide objectives to bring front line staff together to manage crisis. the new modular hub will see staff from rcht’s safeguarding service (adults, children and midwifery); psychiatric liaison; complex care and dementia; addaction; shelter; seap advocacy; child and adolescent mental health in-reach and perinatal mental health teams based in a single location so they can provide prompt, joined up care for people who are vulnerable to, experiencing, or recovering from a mental health crisis. representatives from talking therapies, outlook southwest, police and cft’s home treatment teams will also be invited to use space within the hub. “we are delighted to have been successful in securing new investment for cornwall to deliver this exciting new scheme” said tim francis, nhs kernow’s head of mental health and learning disability commissioning. “all health and care partners are committed to working together to join up mental health services in cornwall so we can better meet the needs of vulnerable people with complex mental health issues. this funding will help us to achieve this by setting up an integrated clinical hub”. “impact will help us to build on the services which are already available to patients in crisis when they attend the emergency department” added lerryn hogg, rcht’s manager for mental health, learning disabilities and autism. “we recognise that many of these people have a range of complex needs which require innovative interventions from different services to support them. creating a single location will help strengthen joint working by breaking down barriers which will, in turn, enable us to develop more effective crisis pathways and improve patient experiences. “the teams will provide a rapid response, assessment and shared care approach to support patients within rcht during crisis. they will also signpost patients to community services, support relapse prevention and help reduce frequent attendances. “i am excited and proud that we now have the opportunity to further integrate our services with colleagues from across the health and social care community and look forward to working together to develop this unique and innovative vision.” announcing the funding jackie doyle-price, minister for mental health and inequalities, said. “the last place anyone experiencing a mental health crisis should be is in a busy emergency department, let alone a police cell. “we are funding a range of innovative local projects that will provide a safety net for those at risk of mental health crisis and make sure they receive the care they need in a safe and secure setting. this builds on our previous grant scheme and will strengthen essential services to help prevent people from reaching crisis point, whilst improving support for those who do.” -- in this section taking time to pause and reflect black books video 2013 most unique getaways newest trends in fashion
